We Have the Answers to Frequently Asked Questions

What are your office hours (when someone from your support staff is available)?*

Monday - Thursday 9am - 6pm
Friday 9am - noon
Saturday 9am - noon
*Please note that therapists and MD’s typically extend their hours beyond these times.

What is the best way to schedule an appointment with a psychologist, therapist, or psychiatrist?

The first appointment for everyone on our professional team is scheduled by a member of our office staff. Follow up appointments are typically scheduled by office staff, but can also be scheduled by your therapist or psychiatrist.

Please call 847/303-1880 press "0."

After each appointment, your therapist or psychiatrist will let you know when he or she would like you to schedule a follow up session. Please note that most follow up appointments with psychiatrists may be as soon as one to two weeks, when medications are first prescribed, and further apart once medications are stable.

Helpful tips on appointments

Our therapists and doctors book up very quickly, so we recommend that you schedule your follow up appointment when you check out.

BPA does not guarantee confirmation calls.  We do our best to make reminder calls, but there may be occasions when we will be unable to do so.  We will be glad to give you an appointment card as a reminder when you schedule your appointment.

Although your psychiatrist may choose to write you a prescription for 3 months or longer, follow up appointments as indicated by your psychiatrist are still required.  Our maximum time between appointments is 3 months (please check with your psychiatrist if you are unsure). If you do not follow up within that time, you may experience a delay in having your prescription refilled.

Please be aware that failure to follow up with a member of our professional team after a 6 month period of time will automatically result in your clinical case being closed, and may require a new evaluation if you choose to see your therapist or psychiatrist again.

BPA requires 24 hours notice for cancellations or the patient will be billed (please consult our fee schedule).

PLEASE CHECK IN WITH THE RECEPTIONIST when you arrive for an appointment. If you do not inform the receptionist you are here, the therapist or doctor will be unaware you have arrived.

What is the best way to have my prescription refilled?

Refills are called in Monday - Friday 9am - 4pm, excluding Holidays when the office is closed.

NOTE: To have your prescription refilled in the quickest manner, please contact your pharmacy and ask them to fax over a prescription refill request.

Medications (controlled substances) that cannot be called in are:
Adderall (XR), Concerta, Ritalin, Focalin, Dexedrine, Methylin, Metadate, Dextrostat, Vyvanse, Daytrana Patch

These medications require a written prescription that cannot be called in or faxed to a pharmacy and must be filled within 90 days from the date it was written.

BPA psychiatrist is Alex Kondic, M.D.

Dr. Kondic reviews requests several times during the day, when he is in the office, and will either refill the prescription or require an appointment in order for the prescription to be filled.

Mail order prescriptions are written at appointments only. BPA does not fax or call in mail order requests.

All lost, destroyed, altered and/or expired prescriptions may be subject to a $15 rewrite charge per incident and are due when picking up the new prescription. This charge cannot be billed to, nor reimbursed by insurance. Each psychiatrist determines whether there will be a charge requested for a rewritten prescription.

Please provide all of the following information when asking for a prescription to be called in:

*Your name (PLEASE SPELL) and doctor you see.
*Your return telephone number with area code (Please turn off privacy manager for return calls).
*The medication(s) you need refilled (include amounts and dosage(s)).
*A pharmacy telephone number (Illinois pharmacies only).
*If your prescription can not be called into a pharmacy, please specify if you would like to pick it up (at the Hoffman Estates office only) or if you would like to have it mailed to you (include address).

Will insurance cover the cost of treatment?

Although we participate in many insurance plans, participation in a particular plan might vary across our clinical team. Please feel free to contact our business office to discuss this further.

What if I have questions about billing?

If you have questions regarding a bill or statement you received, please contact Julie Perry at 1-847-303-1880 x12. Please note that Ms. Perry is available Monday-Friday between 9:00 am and 2:00 pm.

What are Your Payment Policies Regarding Appointments with teenagers?
